The first thing you need to do is fill out an application. You can either download it off of our website or call and we will mail you one.

Unfortunately we receive a lot of applications daily. Therefore we are unable to call every person to let them know. Please feel free to call us to verify we have received it.

THE APPLICATION SAYS TO MAIL IN "PROOF OF INCOME" WHAT DOES THAT MEAN?
This means your social security benefit statement, pay stub, disability information, etc.

WHAT DOES "DEVELOPMENTALLY DISABLED" MEAN?
This is a disability that you would have had since birth. Example: Autism, blindness, deafness, cerebral palsy, cystic fibrosis, epilepsy, downs syndrome, mental retardation, multiple sclerosis, muscular dystrophy, paralysis, polio quadriplegia, spinal bifida, and other rare afflictions.

WHAT IF I AM NOT ELDERLY OR DEVELOPMENTALLY DISABLED?
That's o.k. We also get special funding for our special program. If you have low or no income and have NO dental insurance you could possibly qualify.

IS D-DENT A DENTAL CLINIC?
NO, we are a dental referring agency. We find Dentist to help you through our program.

HOW DOES THE D-DENT PROGRAM WORK?
Prospective Client needs to fill out an application. If they qualify, they are placed on a waiting list. It is a first come, first serve program. Waiting list can be from 2 months to 3 years. If they do not qualify, we will send a letter of explanation with reason/s why they do not qualify.

We are available Monday - Friday 8:30am to 4:30pm. We are closed Holidays and during lunch from 12:00-1:00. Please leave a message.

WHAT IS THE WAITING PERIOD FOR GETTING DENTAL HELP?
We receive over 1,700 applications a year, it is extremely hard to say exactly. It could be around 2 months to sometimes even 2 years.

THE APPLICATION SAYS TO SEND PROOF OF INCOME FOR EVERYONE IN MY HOUSEHOLD, WHY?
We base income off of EVERYONE in your household. We must have everyone's proof of income before we can process your application.

I RECEIVED MY APPLICATION BACK IN THE MAIL WITH A "MISSING INFORMATION" LETTER, WHAT DOES THIS MEAN?
If you do NOT mail all the correct information and fill in ALL BLANKS on the application we WILL MAIL IT BACK TO YOU. This will make your "wait" longer for dental care, so please make sure you fill it out COMPLETELY, otherwise we will return it to you.
